Press the blue button under the mod title, then wait for 60 seconds, and a slightly different button appears. Press that one , and voila.

I think Jakewx is refering to Lashowe. Most likely culprit is Tomb of Jesste Dal'Kest mod (by Darkkender), if installed.

As I said it happened to me only once while playing vanilla game, I still have yet to play TSLRCM.

I encountered this problem once in (heavily moded) vanilla game. It was the Handmaiden's robe that vanished.
