IIRC, the puzzle actually has two solutions but I'm not sure if the game recognizes them both. Source (somewhere on this page) Quote

Salk Posted May 14, 2016 Posted May 14, 2016 What's confusing is that the game does not explicitly tell the player that each operator can be used once. The dialogue should be tweaked in such way that when you choose an operator, you cannot choose it again for the next number. Quote
