The short answer is create a new row in heads.2da for your head, create a new row in appearance.2da that references the new heads.2da ID, create a new row in portraits.2da which references the appearance.2da ID and your custom portraits. If you plan on releasing something publicly, you'll want to set it up with TSLPatcher so that it is compatible with other mods. There are a multitude of tutorials and posts about it.
