1) During the game, when did the problem begin to occur?
During the cutscene with Squidward and Hanharr it switches to Mira and you get the rundown of her abilities (Mira comes equipped with a rocket launcher, etc) right after clicking that fades out to black screen and game crashes.

I have a save before the Exile opens the door and one before entering the private chambers, same results.
